Field/facility request forms received by the Litchfield Recreation Commission are stored here.

The file names are “yyyymmdd-OrganizationName” where the first 8 characters are the date the request was received by the Commission (year, month, day). All of the forms received are stored here, whether they are pending action, approved, or rejected.
